Community Learning Disability Team is a specialist health service for people with learning disabilities and their carers in Sheffield.
Community Learning Disability Team are a multi-disciplinary team. They offers comprehensive, holistic, specialist health assessments, diagnosis, support and care for people aged 18 and above.
Referrals are accepted from primary care (GPs), secondary care and social care services as well as from service users’ families and voluntary or private sector organisations. To make a referral you will need to fill in a referral form and return it via email to [email protected] or by post to their address.
