Community Mental Health Team is an NHS team that provides support to adults with complex or ongoing mental health issues in Sheffield.
They provide support through eight multi-disciplinary care groups, which means they are made up of staff with different professional backgrounds. They are each partnered with a General Practice, Primary and Community Mental Health Care and the local Voluntary, Community and Social Enterprise organisations.
Support is based on individual needs and you will be fully involved in deciding what will work best for you.
The Community Mental Health Team will regularly review your care plan with you and anyone else you would like to be involved in your care, such as family, loved ones or carers. They will also work closely with other local NHS mental health services to manage any changes in your needs.
Referrals are normally received from GPs and primary care, however they also accept referrals from Sheffield Health and Social Care’s crisis and inpatient services.
