Neuro Case Management Service

Neuro Case Management Service is a specialist community team working with people who are living with the challenges of a long-term neurological condition, their families and carers.

They aim to assist and sometimes co-ordinate services to help people improve how their condition is managed, especially where many services are involved. The service ensures that each individual has a Care Plan for their needs now and in the future.

They can support any adult aged 16 or above who is registered with a GP in Sheffield and has been diagnosed with a neurological condition such as Parkinson’s, Cerebral Palsy, Motor Neurone Disease, Epilepsy or has an acquired or traumatic brain injury.

The Neuro Case Management Service work with patients to access the appropriate care to best support your needs. That means that they are often working behind the scenes co-ordinating other services. They can support patients to access advice on everything from medicines management and accommodation needs, to supporting family or carers.

The Neuro Case Management Service forms part of SHSC’s Long Term Neurological Conditions service which also includes the Neurological Enablement Service and the Sheffield Community Brain Injury Rehabilitation Team.

How to access the service: 

People can refer themselves to the service, or they can be referred by a health or social care professional.

A relative or carer can also refer to the service but they must have your permission to do so.

People referred to the team will initially come through to the Referral Point Unit who will triage the referral.
