Memory Service is run by Sheffield Health and Social Care (SHSC). It offers support to older people who are experiencing memory problems as well as other mental health difficulties, like anxiety or depression. They also provide early assessment for the diagnosis of Dementia.
The support they provide is based on individual need. It can include specialist assessment, prescribing medication, post-diagnosis support and peer support groups.
The team is based in the Longley Centre at the Northern General Hospital, but they also provide community support across the city.
The Memory Service also offer a range of support to carers and ensure that they have everything they need to be confident that their loved one is being cared for.
Telephone lines are open Monday to Friday 8.45am – 4.45pm.
The Memory Service accept referrals for adults aged 65 or over who are seeking an assessment or diagnosis for memory difficulties. They also accept referrals for adults, including any under the age of 65, who have a diagnosis of Dementia and need a cognitive enhancer (medication) follow up. Referrals to the service can be made by GPs, and healthcare professionals working in liaison psychiatry, neurology and geriatrics. Referrals should be made via the Older Adults Community Mental Health Teams. Self-referrals are not accepted.
